I authorize the inspection, diagnosis, and tear down to be done by University Auto Service of my vehicle. I understand that if I do not authorize completion of a repair or service, a fee will be imposed for disassembly, reassembly, or partially completed work. Such fee will be directly related to the actual amount of labor and parts involved in the inspection, tear down, repair and/or service. Diagnostic fees may also apply.
I authorize University Auto Service to purchase and furnish the necessary parts and materials for the inspection, diagnosis, and tear down of my vehicle. University Auto Service is not responsible for any delays caused by the unavailability or delayed availability of parts or materials for any reason. University Auto Service is not responsible for the loss or damage to my vehicle, or articles left therein, in case of fire, theft, or other causes beyond their control. I authorize University Auto Service employees to operate my vehicle on streets, highways, or elsewhere for the purpose of testing, inspecting, and/or diagnosing my vehicle.
Unfortunately, space is limited. Storage fees will be applied 3 days after estimate or tear down. Storage fees will be applied 4 days after completed repairs. Storage fees are $40 per day.
